Central Mountain Options for Long Term Care (CMOLTC) is the Single Entry Point agency for Custer and Fremont counties. Our goal is to provide long-term services to elderly and disabled persons as a means to help people continue to live in community settings as alternatives to nursing home care.
CMOLTC provides case management services to Medicaid eligible and private pay clients for the purpose of enabling persons in need of long-term care to access appropriate long-term care services. Home and Community Based Services (HCBS) administer to the following populations:
Elderly, blind and disabled
Persons living with AIDS
Persons with major mental illness
Persons with brain injury
Persons with spinal cord injury
Children with life limiting illness
Eligibility is two pronged. Clients must meet a Functional Assessment criteria as well as be Medicaid eligible. Clients who meet the qualifications through the Functional Assessment but are not Medicaid eligible, may purchase case management services. Eligibility and service needs are re-determined on an annual basis or at a change of circumstances.
Clients who desire long-term care services shall meet the following criteria:
- The individual shall require skilled, maintenance and/or supportive services.
- The individual has functional impairment in the activities of daily living (ADL), and/or need for supervision, necessitating long-term care services provided in a nursing facility, a residential alternative or the individual's home.
- If the individual has a primary diagnosis of developmental disability or mental illness, the individual's needs are primarily for long-term care services, in accordance with the specific program eligibility criteria.
- The individual receives or is eligible to receive medical assistance (Medicaid) and/or financial assistance under one or more of the following programs Old Age Pensions, Aid to Blind, Aid to Needy disabled, Supplemental Security Income, Colorado Supplemental, a 300% eligible as defined at 8.485.50, receiving long-term care services in a nursing facility or through one of the Home and Community Based Service programs listed above.
